You will fill out the Tree Tops Name Tags Purchase Form with your information. Please print all of the information so none of it is submitted incorrectly to our vendor. There are 4 spots for names in case you would like to order for more than one person in your household and a spot for quantity in case you'd like to order more than one name tag per person.
You will drop off the Purchase Form along with a check addressed to "Image 360" for the total amount at the Business Cabin. (We will not accept cash, check ONLY).
Each name tag is $15.
